INSTRUCTIONS

The Makka Pro International Surf Contest will be held on December 29-30, 2018 at the "Makka Surfing Beach" in Yallahs, St. Thomas, Jamaica. All competitors must complete the Registration and Indemnity Form in order to reserve a spot in the competition. Please be advised that spaces are not confirmed until full payment is made at the Registration Party on Friday December 28, 2018 by 11:00pm.

INDEMNITY

I state that I am familiar with all the hazardous conditions which may exist in connection with surfing events. I am aware that there are hazards that may exist in connection with my participation in the Event, and I voluntarily participate in these activities with knowledge of those hazards.

I hereby assume the risk for any injuries that I may sustain in connection with the Event, and I hereby fully release, indemnify and forever discharge Makka Pro, the Jamaica Surfing Association (JSA) and its Officials, Directors and Sponsors, and their respective agents, volunteers and employees from all claims, damages, actions, suits of judgements that may result from any type of injury or death I may sustain while participating in the Event.

I hereby assign to the JSA the exclusive commercial use of all photographs and photographic reproductions, television broadcast and motion pictures taken of me during the Event, whether in or out of the water, as well as permission to arrange to telecast, show, or print and publish any and/or all pictures taken of me (movie, video, or still) in conjunction with the Event.

I agree to conduct myself in a professional and sportsmanlike manner prior to, during and after the Event and while I am in the vicinity of the Event. I shall not engage in any surfing activity near the contest area, unless authorized to do so, during the Event.

I have had an opportunity to review all the rules and regulations applicable to the competition and understand that any violation of these rules, including any unsportsmanlike conduct, will be cause for penalties, which may take the form of a warning, fine, or immediate disqualification from the Event. These decisions will be taken in accordance with the Rules and Regulations of the JSA by the Contest Director and the JSA Board of Directors.

I agree to compete and appear in competition attire provided by the organizers of the Event from time of issue until completion of competition and during awards presentations.

I hereby verify my agreement to accept any and all risk of injury or dealth arising out of or related to the Event by signing this release form.

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between myself and the JSA and Sponsors of the Event, and their respective directors, shareholders, agents, volunteers, employees, successors, assigns and affiliated organizations. I sign this agreement of my own free will.
